FAA Aviation Medical Certificates
We perform examinations for First, Second and Third Class Medical Certificates for students, general aviation, commercial and ATP pilots flying "N" registered airplanes in Europe, in the USA and around the world.
The complete exam includes vision testing and we request that if you wear contact lenses please remove them before the exam and bring a pair of eyeglasses. If you take medication please bring the correct name of the medication as well as the dose.
If you have seen a physician for a medical problem within the last 3 years please bring the physician's name , address, specialty and date of your problem.
You do not need to be fasting for this exam.

If there are any issues or concerns that require follow-up, we will provide all the assistance necessary to complete this process with the FAA office in Oklahoma and help answer and resolve your questions.
BEFORE your appointment
You will need to complete form 8500-8 online at www.medxpress.faa.gov/
With your email address you will set up an account and then complete the 8500-8 form.
You will be given a CONFIRMATION number. Bring the confirmation number to your appointment.
